Alabama ASA JO Tournament Information Sheet

Items required for entering an ASA JO Girls Qualifier or State tournament.

1. Original ASA Roster, signed by parents. The back of the ASA Roster should be completed and signed by Al Murray—Alabama ASA State Director or an AASA District Commissioner.

2. Each team must have proof of Bollinger Sports Insurance (www.BollingerASA.com).

3. Each player must have a photo ID. 16U & 18U can use driver's license as form of ID.

4. Each coach planning to "coach on the field" must complete and pass Ace Certification and a background check. This can be done on the Alabama ASA web site. (www.alabamaasa.com)

5. Each coach must also have a CDC Certificate showing they completed the Concussion Course on the CDC web site. Information for this can be found at www.alabamaasa.com

Tournament Dates—

June 1-3 ASA 10U, 12U & 14U National Qualifier. Entry Deadline: May 25, 2012. 3 game guarantee. Entry fee $300.00.

July 6-8 AASA Girl's A 10U, 12U, 14U, 16U & 18U State Tournament. Entry deadline June 29, 2012. 3 game guarantee. Entry fee $300.00.

Sports Injury Policy - Alabama ASA Championship Play

Any Junior Olympic Participant (18 and Under) that wishes to compete after sustaining an injury must visit the doctor following the sports-related injury. Alabama ASA requires the player to have an Alabama ASA Doctor's Release Form signed by their doctor before they can return to competition. Some of the injuries that must have a physician's release are listed below:

Concussion or Possible Concussion

Broken/Fractured Bones

Serious Sprains

Any Heat Related Injury

Any other Medical Condition that on-site Medical Personnel/Medical Trainer deems it necessary

The above list is not all encompassing. If the onsite medical personnel believes the injury is of a serious nature and requires further evaluation then a Doctor's Release will be required in that instance also.

Alabama ASA requires the release to be given to an Alabama ASA Staff Member and also a copy forwarded to the Alabama ASA State Office. The release must be in the possession of the staff member at the tournament site prior to the player returning to competition.
